{"id":1233,"date":"2023-07-13T15:21:11","date_gmt":"2023-07-13T15:21:11","guid":{"rendered":"https:\/\/medinaz.com\/blog\/?p=1233"},"modified":"2023-07-13T15:21:12","modified_gmt":"2023-07-13T15:21:12","slug":"endodontic-treatment-in-diabetes","status":"publish","type":"post","link":"https:\/\/medinaz.com\/blog\/2023\/07\/13\/endodontic-treatment-in-diabetes\/","title":{"rendered":"Endodontic treatment in diabetes"},"content":{"rendered":"\n<p>Endodontic treatment in diabetes: How to approach endodontic treatment in diabetic patients.<\/p>\n\n\n\n<h2>Diabetes Prevalence:<\/h2>\n\n\n\n<ul><li>&nbsp; According to CDC, 25.8 million people (8.3% of the US population) had diabetes in 2011.<\/li><li>&nbsp; 1.9 million people aged 20 years or older were newly diagnosed with diabetes in 2010.<\/li><\/ul>\n\n\n\n<h2>Impact of Diabetes:<\/h2>\n\n\n\n<ul><li>&nbsp; Diabetes is the <strong><em>seventh<\/em><\/strong> leading cause of death in the US.<\/li><li>&nbsp; People with diabetes have <strong><em>twice the risk<\/em><\/strong> of <strong><em>death<\/em><\/strong> compared to those without diabetes.<\/li><li>&nbsp; Patients with diabetes requiring endodontic treatment are becoming increasingly common.<\/li><\/ul>\n\n\n\n<h2>Pathophysiology of Diabetes:<\/h2>\n\n\n\n<ul><li><strong><em>Insufficient insulin production<\/em><\/strong> or <strong><em>ineffective insulin utilization<\/em><\/strong> leads to <strong><em>elevated blood glucose levels.<\/em><\/strong><\/li><li><strong><span class=\"has-inline-color has-vivid-red-color\">Type 1 diabetes:<\/span><\/strong> <strong><em>Autoimmune destruction<\/em><\/strong> of pancreatic beta cells results in little to no insulin production.<\/li><li><span class=\"has-inline-color has-vivid-red-color\"><strong>Type 2 diabetes:<\/strong><\/span> <strong><em>Insulin resistance<\/em><\/strong> occurs, where cells become less responsive to the effects of insulin, leading to compensatory hyperinsulinemia. (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7791288\/\" target=\"_blank\" rel=\"noreferrer noopener\">ref<\/a>)<\/li><li>In both types, impaired glucose uptake by cells leads to increased hepatic glucose production and release into the bloodstream.<\/li><li><strong>Hyperglycemia<\/strong> stimulates the release of glucagon, further promoting <strong><em>hepatic glucose production.<\/em><\/strong><\/li><li>Increased blood glucose levels contribute to osmotic diuresis, causing <strong><em>polyuria<\/em><\/strong> (excessive urination) and subsequent <strong><em>polydipsia<\/em><\/strong> (excessive thirst).<\/li><li>Loss of glucose in urine leads to <strong><em>glycosuria<\/em><\/strong>, further contributing to <strong><em>fluid and electrolyte imbalances.<\/em><\/strong><\/li><li><strong>Lipolysis<\/strong> is enhanced, leading to increased fatty acid release and subsequent ketone production, which can result in <strong><em>diabetic ketoacidosis<\/em><\/strong> in type 1 diabetes.<\/li><li>Chronic hyperglycemia causes <strong><em>advanced glycation end products<\/em><\/strong> (AGEs) formation, contributing to tissue damage and diabetic complications in various organs.<\/li><\/ul>\n\n\n\n<h2>Considerations for Endodontic Treatment in Diabetic Patients:<\/h2>\n\n\n\n<ul><li>&nbsp; <strong><em>Well-controlled diabetes without serious complications<\/em><\/strong> makes a patient a candidate for endodontic treatment.<\/li><li>&nbsp; Special considerations apply during <strong><em>acute infections<\/em><\/strong>.<\/li><li>&nbsp; <strong><em>Non-insulin-controlled<\/em><\/strong> patients may require <strong><em>insulin adjustments<\/em><\/strong>.<\/li><li>&nbsp; Surgical procedures may require consultation with the patient&#8217;s physician to adjust insulin dosage, antibiotic prophylaxis, and post-treatment diet.<\/li><\/ul>\n\n\n\n<h2>Glucose Monitoring and Hypoglycemia:<\/h2>\n\n\n\n<ul><li>&nbsp; Patients who self-monitor glucose levels should bring a <strong><em>glucometer<\/em><\/strong> to each visit.<\/li><li>&nbsp; If pretreatment glucose levels are below normal fasting range, a <strong><em>carbohydrate source<\/em><\/strong> may be appropriate.<\/li><li>&nbsp; <strong><span class=\"has-inline-color has-vivid-red-color\">Signs of hypoglycemia<\/span><\/strong> include <strong><em>confusion<\/em><\/strong>, <strong><em>tremors, agitation, diaphoresis, and tachycardia.<\/em><\/strong><\/li><li>&nbsp; Clinicians should obtain <strong><em>accurate insulin and meal history<\/em><\/strong> and consult the patient&#8217;s physician if necessary.<\/li><\/ul>\n\n\n\n<h2>Dental Treatment Planning:<\/h2>\n\n\n\n<ul><li>&nbsp; Well-managed diabetic patients without serious complications can receive any necessary dental treatment.<\/li><li>&nbsp; Patients with serious medical complications may require a modified treatment plan.<\/li><li>&nbsp; <strong><em>Prophylactic antibiotics<\/em><\/strong> may be prescribed for patients with <strong><em>difficult-to-control diabetes and invasive procedures.<\/em><\/strong><\/li><li>&nbsp; <strong><em>Local anesthesia is generally safe<\/em><\/strong>, but caution should be exercised for patients with concurrent hypertension, recent myocardial infarction, or cardiac arrhythmia.<\/li><\/ul>\n\n\n\n<h2>Impact on Endodontic Treatment:<\/h2>\n\n\n\n<ul><li>\u00a0 <strong><span class=\"has-inline-color has-vivid-red-color\">Inadequate diabetic control<\/span><\/strong> may increase the risk of <strong><em>oral infections<\/em><\/strong>, including <strong><em>dental pulp infection.<\/em><\/strong><\/li><li>\u00a0 The success of endodontic treatment may be influenced by diabetes, particularly in cases with pretreatment periradicular lesions.<\/li><li>\u00a0 Patients with diabetes and other systemic diseases may benefit from referral to an endodontist for treatment planning.<\/li><\/ul>\n\n\n<div class=\"ub-styled-box ub-notification-box\" id=\"ub-styled-box-ebfc561e-97a9-4880-82e9-f6e92750b927\">\n\n\n<p>You may also like these <strong><a href=\"https:\/\/medinaz.com\/blog\/category\/dental-notes\/\" target=\"_blank\" rel=\"noreferrer noopener\">Free Notes<\/a><\/strong><\/p>\n\n\n<\/div>","protected":false},"excerpt":{"rendered":"<p>Endodontic treatment in diabetes: How to approach endodontic treatment in diabetic patients.
