It has both acute and chronic stages in pulp.<\/p>\n\n\n\n<h2>Characteristics of Pulpal Pain:<\/h2>\n\n\n\n<ul><li><strong><em>Rapid onset<\/em><\/strong> of pain triggered by <strong><em>sudden temperature changes, sweet or acidic foods<\/em><\/strong>.<\/li><li><strong><em>Pain persists<\/em><\/strong> even after the <strong><em>removal of the stimulus<\/em><\/strong>.<\/li><li>Spontaneous pain can occur and is described as <strong><em>sharp, piercing, intermittent, or continuous.<\/em><\/strong><\/li><li><strong><em>Pain worsens when bending down<\/em><\/strong> or <strong><em>lying down<\/em><\/strong> due to changes in intrapulpal pressure.<\/li><li><strong><em>Referred pain<\/em><\/strong> may be present, indicating pain felt in other areas associated with the affected tooth.<\/li><li>In <strong><em>later stages<\/em><\/strong>, pain becomes <strong><em>severe, boring, throbbing<\/em><\/strong>, and <strong><em>intensifies with hot stimuli<\/em><\/strong>.<\/li><li>Severe pain may keep the patient <strong><em>awake at night<\/em><\/strong>.<\/li><li><strong><em>Relief<\/em><\/strong> of pain can be achieved by using <strong><em>cold water<\/em><\/strong>, and patients may come to the dental office with a jar of ice water.<\/li><\/ul>\n\n\n\n<h2>Etiology of Irreversible Pulpitis:<\/h2>\n\n\n\n<ul><li>Pulpitis, commonly caused by <strong><em>bacterial invasion<\/em><\/strong> through dental caries.<\/li><li>Pulpal inflammation can be triggered by <strong><em>chemical, thermal, or mechanical<\/em><\/strong> injuries to the pulp.<\/li><li>If left untreated, reversible pulpitis can progress and develop into <strong><em>irreversible pulpitis<\/em><\/strong>.<\/li><\/ul>\n\n\n\n<h2>Diagnosis of Irreversible Pulpitis:<\/h2>\n\n\n\n<h3>Visual examination and history:<\/h3>\n\n\n\n<ul><li>Previous symptoms may be evident upon examination of the affected tooth.<\/li><li><strong><em>Deep cavity<\/em><\/strong> involving the pulp or <strong><em>secondary caries<\/em><\/strong> under restorations may be observed.<\/li><\/ul>\n\n\n\n<h3>Radiographic findings:<\/h3>\n\n\n\n<ul><li>X-rays can reveal the <strong><em>depth and extent of caries<\/em><\/strong>.<\/li><li>In advanced stages of pulpitis, there may be a <strong><em>slight widening in the periapical area<\/em><\/strong>.<\/li><\/ul>\n\n\n\n<h3>Percussion:<\/h3>\n\n\n\n<p>The tooth may be <strong><em>tender when tapped (percussed)<\/em><\/strong> due to increased intrapulpal pressure caused by inflammatory tissue.<\/p>\n\n\n\n<h3>Vitality tests:<\/h3>\n\n\n\n<p><strong>Thermal test:<\/strong><\/p>\n\n\n\n<ul><li>Hyperalgesic pulp (inflamed pulp) is <strong><em>more sensitive to cold stimulation<\/em><\/strong> compared to a normal tooth.<\/li><li><strong><em>Pain may persist<\/em><\/strong> even after the removal of the irritant.<\/li><li>As pulpal inflammation progresses, the response intensifies with heat and relieves with cold due to their effects on blood vessels.<\/li><\/ul>\n\n\n\n<p><strong>Electric test:<\/strong><\/p>\n\n\n\n<ul><li>In the initial stages, less current is required to generate a response.<\/li><li>As the pulp tissue becomes more necrotic, a higher current is needed to elicit a response.<\/li><\/ul>\n\n\n\n<figure class=\"wp-block-image size-full\"><img loading=\"lazy\" width=\"1024\" height=\"768\" src=\"https:\/\/medinaz.com\/blog\/wp-content\/uploads\/2023\/07\/Irreversible-Pulpitis-Diagnosis.jpg\" alt=\"Irreversible Pulpitis class=\"has-inline-color has-black-color\">Irreversible Pulpitis Diagnosis<\/span><\/strong><\/figcaption><\/figure>\n\n\n\n<h2>Treatment:<\/h2>\n\n\n\n<ul><li><strong>Pulpectomy<\/strong>, i.e. root canal treatment<\/li><\/ul>\n\n\n\n<p><strong>Also know:<\/strong><\/p>\n\n\n<div class=\"ub-styled-box ub-bordered-box\" id=\"ub-styled-box-0af5dc36-3aa8-46e1-b94f-858125a47f97\">\n\n\n<p><strong>Symptomatic Irreversible Pulpitis:<\/strong><\/p>\n\n\n\n<ul><li>It is a <strong><em>clinical diagnosis<\/em><\/strong> indicating that the <strong><em>inflamed pulp is unable to heal<\/em><\/strong>.<\/li><li>Teeth with symptomatic irreversible pulpitis exhibit <strong><em>intermittent or spontaneous pain.<\/em><\/strong><\/li><li>Heightened and prolonged episodes of pain can be triggered by rapid exposure to dramatic temperature changes, especially cold stimuli.<\/li><li><strong><em>Minimal or no changes <\/em><\/strong>may be observed in the <strong><em>radiographic appearance<\/em><\/strong> of the surrounding bone.<\/li><li><strong><em>Advanced<\/em><\/strong> irreversible pulpitis may show <strong><em>thickening of the periodontal ligament<\/em><\/strong> and evidence of pulpal irritation such as calcification in the pulp chamber or root canal space.<\/li><li>Deep restorations, caries, pulp exposure, or other insults to the pulp may be present.<\/li><li>If left untreated, symptomatic irreversible pulpitis can progress to <strong><em>pulp necrosis.<\/em><\/strong><\/li><\/ul>\n\n\n<\/div>\n\n\n<p><meta charset=\"utf-8\"><\/p>\n\n\n<div class=\"ub-styled-box ub-bordered-box\" id=\"ub-styled-box-9cc3517d-eb81-43ef-aca2-5f1ee565dfe8\">\n\n\n<p><strong>Asymptomatic Irreversible Pulpitis:<\/strong><\/p>\n\n\n\n<ul><li>It is a clinical diagnosis indicating that the inflamed pulp is unable to heal.<\/li><li>Despite the <strong><em>inflammation<\/em><\/strong>, the <strong><em>patient does not experience any symptoms<\/em><\/strong>.<\/li><li>Deep caries may not produce symptoms even if it extends into the pulp.<\/li><li>Without treatment, the tooth may eventually become symptomatic or the pulp may necrose.<\/li><li><strong><em>Endodontic treatment should be performed promptly<\/em><\/strong> in cases of asymptomatic irreversible pulpitis to prevent the development of symptoms or pulp necrosis that could cause severe pain and distress to the
