seizures.<\/p>\n\n\n\n<p>Gabapentin was originally planned as a <strong>spasmolytic<\/strong> but it was found to be more effective as an <strong>antiseizure drug<\/strong>.<\/p>\n\n\n\n<p><strong>Pregabalin<\/strong> is another GABA analog and closely related to gabapentin. This drug has been approved for both antiseizure activity and for its analgesic properties.<\/p>\n\n\n\n<h2><span class=\"ez-toc-section\" id=\"Gabapentin_Mechanism_of_action\"><\/span><strong>Gabapentin Mechanism of action<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Gabapentin <strong>does not<\/strong> act directly on GABA receptors in spite of their close structural resemblance to GABA.<\/p>\n\n\n\n<p>They may, however, modify the synaptic or nonsynaptic release of GABA. An increase in brain GABA concentration is observed in patients receiving gabapentin.<\/p>\n\n\n\n<p>Gabapentin is transported into the brain by the <strong>L-amino acid transporter<\/strong>.<\/p>\n\n\n\n<p>Gabapentin binds avidly to the <strong>alpha-2-delta<\/strong> subunit of <strong>voltage-gated Ca2+ channels<\/strong>.<\/p>\n\n\n\n<p>This appears to underlie the main mechanism of action, which is <strong>decreasing Ca2+ entry<\/strong>, with a predominant effect on presynaptic N-type channels.<\/p>\n\n\n\n<p>A decrease in the synaptic release of glutamate provides the antiepileptic effect.<\/p>\n\n\n\n<p>In addition, gabapentin may exert an <strong>analgesic effect<\/strong> by <strong>activating descending inhibitory noradrenergic pathways<\/strong> that regulate neurotransmission of pain signals in the dorsal horn of the spinal cord. <\/em><strong>does not bind<\/strong> to <strong>plasma proteins<\/strong> and is <strong>excreted unchanged<\/strong> through the kidneys.<\/p>\n\n\n\n<p>Reduced dosing is required in renal seizures<\/strong><\/p>\n\n\n\n<p>Gabapentin is considered to be a <strong>first line drug<\/strong> for neuralgic pain due to <strong>diabetic neuropathy<\/strong> and <strong>postherpetic neuralgia<\/strong>.<\/p>\n\n\n\n<p>It has some <strong>prophylactic effect in migraine<\/strong> and is an alternative drug for <strong>phobic states<\/strong><\/p>\n\n\n\n<p>It is also helpful to reduce pain related to <strong>multiple sclerosis<\/strong><\/p>\n\n\n\n<p>Gabapentin is also approved for the treatment of <strong>Bipolar disorder<\/strong> and <strong>Generalized anxiety
